The 4Kscore® Test:
Clarity Before a Prostate Biopsy
The 4Kscore® Test is a non-invasive blood test that helps predict the likelihood of finding aggressive prostate cancer (defined as Gleason score ≥7 / Grade Group ≥2) before a biopsy decision.
The test provides a probability score ranging from 1 to 95, which reflects your chance of finding aggressive prostate cancer on biopsy. A 4Kscore result of less than 5 suggests a very low likelihood of aggressive prostate cancer.
The 4Kscore Test offers you and your healthcare provider dependable, data-driven precision in prostate biopsy decision making:
- Supported by two U.S. clinical validation studies.
- Evaluated in more than 100 published independent studies.
- Utilized by healthcare providers in over half a million men considering biopsy.
- In a major 2024 medical review of leading biomarker tests for assessing the likelihood of finding aggressive prostate cancer (blood and urine tests), the 4Kscore Test was ranked highest in diagnostic accuracy.1

The 4Kscore® Test
Validation and Benefits
The 4Kscore Test is highly accurate, allowing many men to safely avoid a prostate biopsy.
Reduces the need for biopsies in low-risk men
Studies show The 4Kscore Test can potentially help avert a significant number of avoidable biopsies, especially in low-risk men.2
Targets clinically significant cancers
4Kscore helps to focus on aggressive cancers that need treatment, while sparing low-risk cases from invasive procedures.
Endorsed in guidelines
4Kscore is included in U.S., Canadian, and European guidelines for assisting biopsy decisions, including:
- Early detection of prostate cancer: AUA (American Urological Association) / SUO (Society of Urologic Oncology) Guideline2
- The National Comprehensive Cancer Network (NCCN) Guidelines for Prostate Cancer Early Detection3
Who Should Consider The 4Kscore® Test?
Men who meet the criteria below:
- Age 45 years and older
- Age-specific elevated PSA:
- 45-54 years old – total PSA ≥ 2 ng/mL
- 55-75 years old – total PSA ≥ 3 ng/mL
- ≥76 years old – total PSA ≥ 4 ng/mL
- Abnormal digital rectal exam (DRE) results, regardless of PSA levels.
- No previous diagnosis of prostate cancer
